Dry Eye Treatments
Intense Pulsed Light (IPL) Therapy
For many patients, dry eye isn’t caused by a lack of tears — it’s caused by blocked and inflamed oil glands (Meibomian Gland Dysfunction), which standard treatments like drops simply cannot fix.
Intense Pulsed Light (IPL) therapy is one of the most advanced and effective treatments available for this type of dry eye.
IPL works by delivering controlled pulses of light to the eyelid area, helping to:
- Reduce inflammation around the eyes
- Unblock and restore normal function of the oil glands
- Improve the quality and stability of your tear film
Clinical studies show that up to 70–90% of patients experience significant improvement in symptoms following a course of IPL treatment, particularly those who have not responded to conventional therapies.
At Cacoullis Opticians, IPL is used as part of a targeted, medically-led treatment plan, ensuring it is only recommended when clinically appropriate.
Blepharoexfoliation Procedure: Nu Lids Pro/BlephEx
Our optometrist will perform a Nu Lids Pro treatment in order to remove the build of debris, scurf and crusting on the eyelid margins. These contribute to inflammatory eye lid disease such as blepharitis, styes and dry eye syndrome. The Nu Lids Pro treatment is comfortable, fast, effective and highly affordable.

Zocular Eyelid System Treatment (ZEST)
ZEST is an in-office lid cleaning and debridement system using a natural okra-based gel. The procedure is quick and painless, bringing immediate relief to symptoms of irritation and dry eye. It works by gently exfoliating the eyelids to restore the natural state of the eyelids. By cleaning the eyelid margins which opens up the meibomian gland, it helps release oils into the eye that are required for a healthy tear film and reduce symptoms of dry eye. After the in-office treatment, we recommend gentle and effective home treatments to clean and keep the bacteria away from the lids that are anti-microbial, anti-inflammatory and antioxidant.

Meibomian Gland Expression
Meibomian gland expression refers to the process of stimulating the meibomian glands, which are located in the eyelids, to release the oils (lipids) that help form the tear film and keep the eyes lubricated. These glands play a crucial role in eye health, as the oils they secrete prevent the evaporation of tears, keeping the eyes moist and preventing dryness.

Eyelid Debridement
Eyelid debridement refers to a procedure in which the outer layers of the eyelid are gently cleaned or scraped to remove debris, dead skin cells, or other materials that may be contributing to eyelid or eye health problems. This procedure is typically done when there are conditions affecting the eyelid or lash line, such as:
- Blepharitis (inflammation of the eyelid),
- Meibomian gland dysfunction (MGD)
- Chronic dry eye or conjunctivitis.
The goal of eyelid debridement is to help restore the health of the eyelids, improve the function of the meibomian glands, and reduce symptoms of irritation or infection.

Punctal Plugs
Punctal plugs are small devices inserted into the tear ducts (puncta) of the eyes to help treat dry eye disease by blocking the normal drainage of tears. This increases the amount of moisture that stays on the surface of the eye, helping to alleviate symptoms of dryness, irritation, and discomfort. Punctal plugs are typically used in cases where other treatments, such as lubricating eye drops or oral medications, are insufficient.
Punctal plugs are inserted into the small openings at the corner of the eyelids, called puncta, which are the drainage points for tears. Normally, tears drain into the nasal cavity through the puncta. By blocking the puncta with plugs, tear drainage is reduced, and more moisture stays on the eye’s surface, improving lubrication and reducing dryness.
Benefits of Punctal Plugs:
- Improved Tear Retention: By blocking tear drainage, punctal plugs help retain more tears, which can be beneficial for people with dry eye disease.
- Less Dependence on Artificial Tears: Punctal plugs can reduce the need for frequent use of lubricating eye drops.
- Quick and Effective Relief: Most people experience relief from dry eye symptoms soon after the plugs are inserted.

Lacrimal Syringing
Lacrimal syringing is a medical procedure used to assess and treat blockages in the lacrimal (tear) drainage system, which consists of the puncta (tiny openings in the eyelids), the lacrimal sac, and the nasolacrimal duct. This procedure involves flushing a sterile fluid through the tear ducts to check for blockages, and it may also be used to clear minor obstructions in the system.
Lacrimal syringing is typically done for patients who present with symptoms such as:
- Excessive tearing or watery eyes (epiphora)
- Recurrent eye infections or conjunctivitis
- Mucus or pus in the corner of the eye
- Chronic eye irritation or discomfort due to poor tear drainage

In-Practice Dry Eye Treatments for Dry Eye Syndrome: Blepharitis/Meibomian Gland DysfunctionA Dry Eye clinic consultation is highly recommended in order to accurately diagnose the underlying cause of “dry-eye” related symptoms.

Tier 2 Treatment
Zocular Eyelid System Treatment (ZEST) for Dry Eye
ZEST is an in-office lid cleaning and debridement system using a natural okra-based gel. The procedure is quick and painless, bringing immediate relief to symptoms of irritation and dry eye.

Zest (Zocular Eyelid System Treatment) works by gently exfoliating the eyelids to restore the natural state of the eyelids.
Eye hygiene includes keeping the eyelids, eyelid margin and lashes clean. Dirt and debris on the edges of your eyelids can lead to symptoms of itching, burning and gritty eyes.
With every blink, dirt and debris may enter the tear film and coat the eye, leading to redness and irritation.
Poor eye hygiene can lead to demodex mites in the hair follicles. Demodex can exacerbate inflammation by blocking the oil glands at the base of your eyelashes. ZEST is an effective treatment for killing demodex mites, removing dirt, debris and relieving inflammation.
The ZEST procedure incorporates Zocular’s use of okra to address the root cause of dry eye symptoms. The polysaccharides in okra plants are used in a special mixture to clean the eyelids, targeting the oily residue, bacteria and dirt that are often at the root of itchy, dry eyes. After you undergo the ZEST treatment, your optometrist will be able to recommend ways to keep your eyelids clean in order to manage your dry eye symptoms.
